The dystrophin gene

The dystrophin gene (also known as DMD) (OMIM 300377) has been identified by positional cloning in 1986 on chromosome X (Xp21.2) (Monaco et al., Koenig et al.). It is by far the largest known gene: 2.6 million base pairs (bp) consisting of almost 0.1% of the human genome or about 1.5% of the entire X chromosome. This very large gene is highly fragmented into 79 exons and introns of variable size ranging from 107 bp (intron 14) to >200kb (intron 44). It is transcribed in a 14 kb mRNA, and the 11kb cDNA encodes a 3685 amino acid protein of 427 kDa.

There are multiple gene promoters and alternatively spliced isoforms (Ahn and Kunkel). Expression of the full-length dystrophins is controlled individually by three promoters located in the 5'-end region of the gene (320kb upstream of exon 2) that are active in neurons (the Brain or Cortical promoter, Dp427c) (Nudel et al.), in muscle and glial cells (the M promoter, Dp427m), and in Purkinje cells in the brain (the P promoter, Dp427p) (Gorecki et al.). Each of the 14-kb mRNA transcribed from these promoters consists of a unique first exon and a common set of 78 exons. Recent data support the fact that the lymphocyte promoter (Dp427l) located >500kb 5' of the cortical promoter (Dp427c) described in 1994 (Nishio et al.) should no longer be considered a bona fide promoter of the human dystrophin gene (Wheway and Roberts).

Four amino-terminally truncated forms of dystrophin are transcribed from internal tissue-specific promoters located further downstream. Each of these promoters uses a unique first exon that splices into exons 30, 45, 56, and 63, respectively, to generate smaller proteins, named according to their molecular weights: Dp260 (retina) (D'Souza et al.), Dp140 (brain, retina, kidney tissues) (Lidov et al.), Dp116 (adult peripheral nerves) (Byers et al.), and Dp71 (Lederfein et al.). Dp71 is the major product of the dystrophin gene in brain and many other non muscle tissues. However, it is not expressed in skeletal muscle. In addition to these isoforms, the dystrophin gene produces many other isoforms generated through tissue-specific alternative splicing events or by using an alternative poly(A)-addition site in intron 70 (Dp40 isoform) (Feener et al.). These events generate further protein diversity and account for the complex expression regulation of the tissue-specific functions.

The X-linked dystrophinopathies include a spectrum of muscle diseases caused by mutations that lead to quantitative and qualitative disturbances in the expression of dystrophin. Dystrophinopathies comprise three distinct allelic entities, Duchenne muscular dystrophy (DMD; #310200), Becker muscular dystrophy (BMD; #300376), and X-linked dilated cadiomyopathy (XLDCM; #302045), a rare allelic form in which dystrophin expression or function is affected mostly or exclusively in the heart (Muntoni, F). The large size of the dystrophin gene contributes significantly to its high rate of spontaneous mutation (1 in 10,000 sperm or eggs). These mutations consist in large deletions and duplications of one or more exons (~70%) and small lesions (~30%).
